The H.O.P.E. Network provides Homemaker Personal Care services to ensure that all people with developmental disabilities and their families have equal opportunities to live, learn, work and thrive in their communities. We provide support for individuals with developmental disabilities to promote independence and opportunities throughout their lives. The H.O.P.E. Network gives the gift of hope, creates change and finds solutions.

Residential Home Placement (24/7 Supports)

A safe, welcoming, comfortable place to call home is essential to living a life of independence.

While structured differently to best meet each individual needs as well as to comply with state regulations, all of our homes offer high quality services with the shared goal of supporting individuals to thrive at home and in the community.

Individualized Supported Living/Congregate – one to four individuals share a residential home. Individuals have their own bedroom tailored for their needs and have a voice in choosing their housemates. They share living expenses such as rent, utilities, cable TV, and groceries.

Homemaker Personal Care (HPC)

The H.O.P.E. Network’ Homemaker Personal Care (HPC) program meet the specific needs of every individual we serve. All services provided, are followed by each person’s Individual Service Plan (ISP) to help each of the individual we serve achieve their maximum level of functioning. 

Our staff have been properly trained to empower the people we serve to make positive choices and meet their goals. Our agency focuses on safety, enhanced communication skills and development, strengthening independence, and community integration. We believe the more connection a person has in his/her community, the more likely they are to be successful in the community.

Support services are individualized for each person, however some common areas of support and skill development include but are not limited to:

Personal Care/Hygiene
Bed & Bath Aide
Dressing
Household chores
Meal Preparation
Medication Reminders/ Administration
Scheduling/appointments
Errands & Desired Outings
Budget management
Social skill building

The H.O.P.E. Network’ also assists each of its individuals with: 

Selecting and moving into a home;
Acquiring household furnishings;
Common daily living activities;
Becoming a participating member in
community life as well as other supports.

 Shared Living

The H.O.P.E. Network Shared Living Program places your loved one with a welcoming individual/ family in their community for long or short-term, in-home placement.  All of our shared living providers have over 10 years of experience in the caregiving/ assisted living industry. All of our families provide a safe and nurturing environment inside and outside of their homes.  Each individual will have access to their own room as well as all of the common areas of the home. They are given the option to decorate their room to their liking. Our shared living provider will integrate your loved one into their home and community life. The H.O.P.E. Network is there every step of the way to provide support to both your loved one and the family that will be caring for them. 

Supports Oversight

Our house managers oversee the daily operations for the clients they serve to ensure that each of their needs are met. Each home has a dedicated House Manager that works closely with the local office staff to provide a comfortable living environment, work on the goals and objectives of each individual daily, and integrate the individuals we support in their local communities.
